When you or your loved one is discharged from one of our programs, you may have questions or need additional resources. At PrairieCare, we understand that mental health is a journey, and we are here to help you every step of the way.

Patient Financial Information
  • Financial Information: Access details on insurance details and coverage, including our price estimator tool and the financial assistance application. 
  • Billing Page: Pay the bill associated with the services you or your child revisited. This includes outpatient services, Intensive Outpatient Programs, Partial Hospitalization Programs, and inpatient hospital.
Medication Refills

Intensive Outpatient Program (IOP) and Clinic Services

  1. Please call your pharmacy one week in advance and request a refill directly from a pharmacy staff member.
  2. After calling your pharmacy, please call (952) 826-8475 to schedule a follow-up appointment to ensure we can approve your refill.

To obtain refills, we ask that you:

  • Have a follow-up appointment scheduled.
  • Have been seen within the last three months (children) or six months (adults) or as directed by your provider.

*Requests are processed Monday-Friday during clinic business hours.

Controlled medications have additional requirements and may take 3-5 business days to process. Please contact your pharmacy directly and speak to a pharmacy team member to verify if you have a fillable prescription on file.


 

Partial Hospitalization Program (PHP)

  • Patients discharging from a Partial Hospital Program (PHP) will receive a 30-day prescription of currently prescribed medications with 0 refills. It is important that patients attend their follow up appointments for medication management following discharge due to PHP prescribers not being able to provide refills.

 

Inpatient Hospital

  • Patients discharging from the Inpatient Hospital will receive a 30-day prescription and 0 refills for medications that were started during hospitalization or for medications that required changes to the dose. It is important that patients attend their follow up appointments for medication management following discharge due to the hospital not being able to provide refills.
Release of Information (ROI) Forms 

Filling out an ROI form provides us with your consent to share information about you and your care. Please select the document that best fits your needs from the options below. If you are unsure, complete the standard form.  

Email the completed form to medicalrecordsrequest@prairie-care.com. You can also fax the form to 763-367-7599 or drop it off at the nearest PrairieCare location.  

How to Make Your Home a Healing Space for Your Loved One

When your loved one is discharged from a high level of care or is continuing their treatment journey, supporting them along the way is one of the most important things you can do. 

 

We encourage you to follow the safety plan your treatment team created. You can continue to update your plan as you continue treatment that incorporates the following aspects:

Safety concerns
Stressors and triggers
Warning signs

Coping strategies
Who can support
What you can do to support

Safety Environment Plan

Safely Store: Sharp Objects

  • Store all sharp objects in a safe and secure location within the home (lock box or other locked device). 
  • Routinely check household for sharp objects the client can access. 
  • Initiate and have ongoing conversation with your child about potential items that may be a safety concern to them.

Safely Store: Medications

  • A parent, guardian, or adult must be administering all medication.
  • Remove unneeded and expired medications from your home. Information on safe disposal of drugs is available from the Poison Control Center at 800-222-1222. 
  • Keep only nonlethal medication quantities on hand. Speak with pharmacist or prescribing physician to determine the safe amount. 
  • Secure all medications in a lock box.  
  • Secure alcohol in locked space or off premises. 
  • Secure car keys away from your child’s access.

Safely Store: Firearms

  • Temporarily leave with a relative or friend. 
  • If stored in the home, secure the firearm in a lock box. 
  • Disassemble the firearm and store a key component separately or away from the home. 
  • Utilize a self-storage rental unit. 
  • Store at a pawn shop or gun range. 

Immediate Mental Health Support Options  

If you or a loved one is experiencing severe mental health symptoms, you don’t have to spend hours in the ER waiting room. Our Family First admissions process provides a fast route to immediate psychiatric treatment at our hospital for anyone 35 and younger.

Parents and young adults can call 952-826-8424 anytime to get emergency support.  

If you or someone you love is experiencing suicidal thoughts, go to the nearest ER or call the National Suicide Prevention Lifeline at 988.
