Emergency and Immediate Care Options
The Family First admissions process at PrairieCare allows patients who are 35 and younger with need immediate care for severe mental health symptoms to come directly to our inpatient hospital without a prior visit to the ER. Parents and young adults can call 952-826-8424 to talk with our assessment and intake team at any time. Learn more.
Make a Referral
Our Assessment and Intake team can help schedule a service at any PrairieCare location by calling 952-826-8475 or you can make a referral by clicking the link below if you are a professional. Please note if you are sending a referral via faxed reports, you do not need to complete the online referral form. Results of the mental health screening will determine the recommended level of treatment care and provide guidance for next steps. We look forward to talking with you!
