We offer no-cost mental health screenings for all.

Each person experiences mental health challenges differently, that’s why we offer individualized care for each patient. PrairieCare’s trained Assessment & Intake Department (AID) provides no-cost mental health screenings over the phone to anyone seeking mental health services. Mobile Admissions Process (MAP)

Providing No-Cost Mental Health Assessments On Site or Via Telehealth

If you believe your patient needs admission to a psychiatric hospital, partial hospital setting, or another level of care, contact PrairieCare’s Mobile Admissions team. A member of the team will arrive on site at your location or virtually within two hours to provide an assessment and facilitate admission to the right program.

Here’s How It Works:

Make a Call: Hospital staff calls the Mobile Admissions team at 952-826-8424 when a patient in care is experiencing mental health concerns.

Assessment Occurs: Within two hours, a member of our Mobile Admissions team arrives at your location (within 30 miles of PrairieCare’s hospital) or is available via telehealth to complete a mental health evaluation with the patient. Assessors are on call, in person, Monday–Friday, 8:00 am–4:00 pm, or telehealth, seven days a week, 8:00 am–4:00 pm.

Treatment Is Initiated: The Mobile Admissions team determines the best level of care and starts the admissions process as needed.

Family First Admissions Process: Immediate Psychiatric Care

The Family First admissions process allows patients 35 and younger who need immediate care for severe mental health symptoms to come directly to our inpatient hospital without a prior visit to the ER. Parents and young adults can call 952-826-8424 and get help in three easy steps. Learn more.

  • In case of a mental health crisis, call 988 or seek the nearest emergency room.
  • For physical health emergencies, call 911 or seek the nearest emergency room.

Screenings are not full psychiatric evaluations, but are used to determine appropriate next steps to address an individual’s needs. This may include, but is not limited to: outpatient clinic services, intensive outpatient programs, partial hospital, inpatient hospital, residential treatment, or substance abuse treatment.
